How Hospital Discharge Planners Help Patients

Discharge planners help patients acquire the resources they need to safely recover after surgery and medical procedures. However, helping the patient get access to care after hospitalization is only part of the work a Medical Discharge Planner does. They also provide financial, social, and psychological resources to the patient and family.

Hospital Discharge Planners coordinate with medical staff to facilitate the discharge plan, keep written record of the plan, and help track the the patient’s care. They also link the patient to services after discharge and provide information, encouragement, and assistance to support patient recovery and improved functioning.

The Discharge Planners role is to take time to hear the highly individualized needs of each patient, and provide safeguards at home for care. They also know how to access services, and can help the patient find home care upon discharge.
