A home care assessment evaluates your loved one to help identify any care needs and potential home hazards. It's usually performed by a licensed healthcare professional, such as our Director of Nursing or medical social worker. The home care assessment will include personal care recommendations and home safety improvement recommendations to help improve the environment, such as installing handrails in the bathroom or ensuring your house has adequate lighting and medication management.

An elderly home care assessment can be an important preventive measure, mainly because older people living alone are prone to falls and serious injury. According to the National Council on Aging, one in four Americans age 65 and older falls every year, and falls are the number one cause of injury-related death for older adults.
