Social Workers in Plano / North Dallas Provide Patient Education on Treatment Options and Help Coordinate Services After Hospital Discharge

Hospital social workers may not see every hospitalized patient, but many of their services come from a doctor or nurse referral. Social workers assess the patient’s needs, and will intervene as needed. The assistance they provide may vary depending on the patient’s needs. For example, some patients may need help connecting with services in their community, such as drug treatment support. Other patients may need help understanding their medical condition, and navigating their hospital stay.

Social workers also provide patient education on treatment options and help coordinate services needed after discharge, such as medical equipment, so the patient can return home. Medical social workers also arrange grief counseling or other mental health services for patients.

BrightStar Care of Plano / North Dallas works closely with discharge planners, physician offices, and social workers to ensure continuation of care at home after hospitalization.
