Social Workers Help People Make the Transition from the Hospital Back to Their Homes and Communities

Healthcare social workers help patients understand their diagnosis and make the necessary adjustments to their lifestyle, housing, or healthcare.

Richmond Social Workers provide information on services, such as home healthcare and support groups, to help patients manage their illness or disease at home. Social workers help doctors and other healthcare professionals understand the effects that diseases and illnesses have on patients’ mental and emotional health. Some healthcare social workers specialize in geriatric social work, hospice and palliative care, or medical social work.

Social Workers:

  • Identify patient needs and communities support available

  • Assess family situations, patient strengths, and support networks to determine if they can support the patients goals

  • Help patients adjust to changes and challenges in their lives

  • Respond to crisis situations such as senior abuse or mental health emergencies

  • Follow up with patients and maintain case files and records

Social workers are helping patients every day in Richmond to cope with a range of challenges in their lives. They also advocate on behalf of their patients in the healthcare environment.

“Licensed clinical social workers (LCSW) are licensed to diagnose and treat mental, emotional, and behavioral issues by providing therapy, developing strategies for changing behavior or coping with difficult situations. They may work as part of a team with doctors and other professionals to develop a comprehensive plan for a vulnerable individual.”

BrightStar Care of Richmond works closely with social workers to ensure continuation of care at .home after hospitalization.
