Hospital Social Workers Play a Key Role in San Francisco and Marin County Patient Care

 
Medical social workers provide the initial screening and evaluation of patients and their families, including psychosocial assessments of patients. They help patients and families to understand their illness and treatment options, as well as the consequences of various treatments or treatment refusal.

Medical Social Workers: 

  • Help patients and their families adjust to hospital discharges and other changes that patients experience, including exploring emotional/social responses to an illness and treatment
  • Educate patients on the roles of health care team members
  • Assist patients and families with communicating to members of their health care team
  • Interpret information
  • Educate patients on the levels of health care (i.e. acute, sub-acute, or home care) Entitlements
  • Community resources
  • Advance directives
  • Facilitate decision making on behalf of patients and families
  • Can employ crisis intervention 
Hospital social workers can also help discover an underlying mental illness, providing or making referrals for individual, family, and group psychotherapy, and educate hospital staff on patient psychosocial issues. They also promote communication and collaboration among health care team members and coordinate patient discharge and continuity of care planning.  Helping patients navigate services and return home safely is a key goal.
