Confusion about medication often results in a return trip to the hospital

I remember one time when my grandmother was discharged from the hospital and misinterpreted the information on her discharge papers.
On the paper it stated, 'low sodium diet.'
When she read those words, she mistakenly assumed it meant that she HAD a low sodium diet and needed to ingest MORE sodium. So, she prepared a bowl of salt and began to eat it! Fortunately, my mom, who is constantly checking on her, found the bowl of salt and quickly corrected my grandmother.
Thankfully!

I can see where individuals easily confuse their discharge papers as well as their medications.
One of the top reasons for hospital readmission is non-compliance.

How can you avoid a return trip to the ER with your elderly loved one? Make sure they understand the importance of being compliant. Set up a med reminder for them and fill their med planner to decrease confusion.
