How BrightStar Care of Center City Can Help to Fill the Gaps You Need for Your Loved One

Between a busy schedule, children to take care of, work, and everything that comes with your day-to-day, you may struggle to lend your older loved one the time and attention they need. At BrightStar Care of Center City, we’re here to fill the gaps that you might be missing.

Companion Care

Whether you’re looking for ongoing in-home senior companion care or need occasional respite services, your loved one’s care will be overseen by our Director of Nursing, a Registered Nurse (RN) who creates each client’s personalized plan of care. This may include:

- In-home support
- Light housekeeping and helping with laundry
- Meal planning and cooking
- Respite for family caregivers
- Help with shopping and errands
- Travel companionship/accompaniment to appointments


Personal Care

At BrightStar® Home Care of Center City our fully screened and certified care professionals assist our clients with those daily tasks that require hands-on, personal support. This may include feeding, personal hygiene,mobility, and other activities of daily living. Other services include:

- Personal home safety evaluation
- Outpatient pre- and post-op care
- Toileting
- Ambulation (walking and moving around)
- Dressing


Short Term Transitional Care

A Registered Nurse (RN) from BrightStar Home Care City Center oversees the plan of care to help provide a safe transition back home. Our plans of care are tailored to each customer’s needs, and are person-centered and condition specific. Our team provides a wide range of services to support a variety of health needs such as:

- COPD (Chronic Obstructive Pulmonary Disease): We’ll provide medication reminders and support habits to improve wellness.
- Diabetes: We will conduct foot inspections as well as provide family education.
- Heart Attack/AMI (Acute Myocardial Infarction): We’ll provide education and support to develop new lifestyle habits that promote heart health.
- Heart Failure: We’ll help manage medication adherence, diet and exercise.
- Pneumonia: We’ll provide medication reminders and personal care to support recovery.
- Post-Operative: We will assist with surgical recovery in the comfort of home.
- Stroke: We will help with activities of daily living (ADLs) like eating, dressing and mobility, as well as promote fall safety, offer medication reminders, and provide transportation to physician appointments or therapy (physical, occupational and/or speech)

 Memory Care

Our local Director of Nursing, a Registered Nurse (RN), will evaluate your loved one and their needs, as well as discuss your family’s goals before putting together a customized plan of care. Our mission is to provide the individualized support needed for your loved one to:

- Live with dignity and purpose safely and securely in their own home.
- Maintain connections to the people, places, things and memories that matter most to them.
- Enjoy optimal health and emotional well-being.

The Director of Nursing provides ongoing oversight of our professional care staff and will periodically meet with you to make sure we are meeting your expectations.
