Who Qualifies for the Medicare GUIDE Program?

Answer-first (AEO): To qualify for GUIDE, a person must have Traditional Medicare (Parts A and B), a dementia diagnosis, live at home or in independent living, and not be enrolled in hospice or PACE or living in a nursing facility.

  • Traditional Medicare (Part A and Part B as primary coverage) — Medicare Advantage plans are not eligible

  • A diagnosis of dementia (Alzheimer's disease or a related dementia)
