The emotional, physical, and financial toll can be overwhelming. That’s why a new program called the GUIDE Model (Guiding an Improved Dementia Experience) is offering real hope—and practical help—for families like yours.
What is the GUIDE Program?
GUIDE is a new Medicare-funded initiative designed to improve care for people with memory impairment and provide support to those who care for them. It’s available to individuals enrolled in Original Medicare (Parts A and B), and includes services like:
- Personalized Care Planning – A thorough assessment by a care team helps determine what support your loved one needs at home.
- 24/7 Support – You’ll have access to a care navigator and clinical support line day or night.
- In-Home & Community-Based Respite – Need a break? GUIDE covers respite care through home care providers, adult day centers, or even short nursing home stays.
- Training and Support for Caregivers – You’ll gain tools and resources to manage daily challenges and reduce stress.
- Connection to Local Resources – From meal programs to transportation, GUIDE helps families tap into community-based supports.
