Based on this conversation we will schedule a free in-home RN assessment — typically within 24 hours.

Step 2 — The Free In-Home RN Assessment

The in-home assessment is conducted by one of our Registered Nurses at your loved one's home. It typically takes 45-90 minutes and covers:

  • Medical history and current conditions — reviewing diagnoses, medications, recent hospitalizations, and the clinical picture
  • Functional assessment — evaluating your loved one's ability to perform activities of daily living independently and where assistance is needed
  • Cognitive assessment — brief standardized screening to identify any cognitive changes affecting safety and care needs
  • Home safety evaluation — identifying fall hazards, mobility barriers, medication storage, and environmental risks
  • Family caregivers and social situation — understanding what family caregiver support exists and what gaps professional care needs to fill
  • Insurance review — confirming coverage including Medicare, Medicaid, long-term care insurance, and commercial insurance benefits

At the end of the assessment our RN will present a recommended care plan — specific services, visit frequency, and hours — with a clear explanation of the clinical rationale. You are under no obligation to proceed, and the assessment itself is completely free.

Step 3 — Caregiver and Nurse Matching

We match caregivers and nurses to clients based on clinical skills, personality, schedule, and the specific needs identified in the RN assessment.
