Who Is Eligible for VA Home Care?
To receive home care services through the Veterans Administration:
Step 1: Schedule a Physician’s Assessment
Start by contacting your local VA Medical Clinic to request an assessment.

Note: Your VA physician will not replace your current primary care doctor unless you choose for them to.
Before the visit ends, you can also request a specific In-Home Provider, such as BrightStar Care of Westfield and Summit, to handle your services in Union County.
Step 2: A VA Community Coordinator Will Contact You
After your assessment, a VA Community Coordinator will reach out to discuss the results and let you know how many hours per week of home care you qualify for. If you did not name a preferred provider, the Coordinator will give you a list of local in-home care options. They will then send your information to the provider you select. Step 3: Your In-Home Care Provider Begins Services
