Rachel, our Director of Community Outreach, spent the day at Genesis Rehab of Bridgeville meeting with residents and staff. Thank you to the Genesis team for welcoming her, and for the work you do every day helping patients get back on their feet.
Days like this remind us how important the move from rehab back home is. A little planning can make that transition safer and less stressful for the whole family.
Coming home after rehab: 4 ways to plan ahead
- Start before discharge. Ask the care team about the expected discharge date and what help will be needed at home.
- Review medications. Get an up-to-date list and make sure you understand any changes.
- Make the home safer. Clear walkways, add grab bars and set up a main-floor bedroom if stairs are hard.
- Line up help early. Having support ready on day one can ease recovery and help prevent setbacks.
