A dementia diagnosis can be overwhelming for both the individual and their family. Coordinating medical care, daily activities, safety, and caregiver support can quickly become challenging.
The CMS Guiding an Improved Dementia Experience (GUIDE) Model is designed to provide more coordinated support for people living with dementia and their unpaid caregivers, helping them access care and resources while supporting their ability to remain safely at home and in their communities. The model began July 1, 2024, and is scheduled to run for eight years.
What is the Medicare GUIDE Model?
GUIDE is a voluntary CMS Innovation Center model that provides participating organizations with a structured approach to dementia care. Services can include care coordination, caregiver education and training, care navigation, 24/7 support access, community-resource connections, and respite services.
Does GUIDE provide free home care?
GUIDE can provide respite services at no cost to an eligible aligned patient, because GUIDE participants cannot charge patients for covered GUIDE services. However, GUIDE is not a blanket Medicare benefit that automatically provides free home care to everyone with dementia. Eligibility and enrollment requirements apply.
How much respite support is available?
For eligible patients, CMS established an annual GUIDE respite-services cap of $2,563 for Performance Year 2025. CMS describes qualifying respite options that may include in-home respite care, adult day programs, and facility-based respite. The amount and service availability can change as CMS updates the model.
Who may qualify?
Generally, the person must have a dementia diagnosis confirmed through the GUIDE program, be enrolled in Medicare Parts A and B with Medicare as the primary payer, and meet specific residence and other program requirements. People enrolled in Medicare Advantage, PACE, or hospice generally do not meet the GUIDE eligibility criteria. Individuals residing in long-term nursing homes or memory care units may also be excluded.
Eligibility should always be confirmed with a participating GUIDE organization.
Can home care be part of GUIDE?
Yes. In-home respite care is one of the respite options CMS recognizes under the model. The actual services, hours, scheduling, and provider arrangements depend on the GUIDE participant and the individual's eligibility and care plan.
How can families get started?
If your loved one has dementia and you are providing unpaid caregiving, ask their healthcare provider or a participating GUIDE organization whether they participate in the program.
