Could your family qualify for up to $2,625 in annual respite-care support for a loved one living with dementia? The Centers for Medicare & Medicaid Services (CMS) GUIDE Model is helping eligible individuals and their caregivers access coordinated dementia care, caregiver education, and temporary relief from caregiving responsibilities.
For families caring for a loved one with Alzheimer’s disease or another form of dementia, this program may offer meaningful support and help make it possible to remain safely at home longer.
At BrightStar Care of Acton/Andover & Lowell, we understand that caring for a loved one with dementia can be rewarding, challenging, and exhausting. Below, we answer common questions about the GUIDE program and how respite care may help your family.
Frequently Asked Questions About the Medicare GUIDE Program
1. What is the CMS GUIDE program?
GUIDE stands for Guiding an Improved Dementia Experience. It is an eight-year CMS initiative launched on July 1, 2024, designed to improve care for people living with dementia and support their unpaid caregivers.
The program promotes coordinated, person-centered dementia care, caregiver education, care navigation, and respite services for eligible participants. Its goal is to help people with dementia remain in their homes and communities longer while reducing the strain on family caregivers.
2. Is there a Medicare benefit for dementia respite care?
Yes. Eligible individuals enrolled in the GUIDE Model may qualify for respite-care services up to an annual limit.
For the July 2026–June 2027 performance year, CMS has set the respite-care cap at up to $2,625 per eligible participant. The amount is not a cash payment to the family. It covers qualifying respite services arranged through the GUIDE program, subject to eligibility and program requirements.
Important: The available amount and qualifying services depend on current CMS rules and the individual’s circumstances.
3. Can Medicare provide 72 hours of free personal or companion care?
Families sometimes hear about the GUIDE program as an opportunity to receive approximately 72 hours of respite care. The actual number of hours depends on the type of services provided, the provider’s rates, and the remaining benefit amount.
For eligible participants, GUIDE respite services may include in-home respite care, adult day center services, or facility-based respite. In-home respite can give a family caregiver time to rest, attend appointments, handle errands, or take a much-needed break.
However, 72 hours of personal or companion care is not a guaranteed benefit for every Medicare beneficiary. Families should confirm the number of covered hours and whether their preferred home-care provider participates in the GUIDE arrangement.
4. Who is eligible for the GUIDE program?
Eligibility is determined through a participating GUIDE provider. Generally, an individual must:
-
Have a confirmed dementia diagnosis.
-
Meet the program’s Medicare and other eligibility requirements.
-
Be aligned with a participating GUIDE provider.
-
Meet the specific criteria for the services being requested.
Additional requirements apply to respite care. For example, eligibility for respite services depends on the person’s dementia severity and caregiving situation. Some residential settings also have special restrictions.
Being enrolled in Traditional Medicare alone does not automatically qualify someone for GUIDE services.
5. Does the GUIDE program have out-of-pocket costs?
CMS does not permit participating GUIDE providers to charge aligned patients cost-sharing for covered GUIDE services, including qualifying respite care.
This does not mean that every service a family needs is automatically free. Services outside the GUIDE program, care beyond applicable limits, or arrangements with providers that are not covered under the program may involve separate costs.
Before scheduling care, confirm the services covered, available respite balance, and any costs that could apply to services outside the program.
6. What services does the GUIDE program offer besides respite care?
The GUIDE Model is designed to support both the person living with dementia and their caregiver. Services may include:
-
Comprehensive assessment: Identifying the individual’s care needs and developing a personalized care plan.
-
Care navigation and coordination: Helping families understand available services and coordinate support.
-
Caregiver education: Providing information, training, and practical strategies for dementia care.
-
Caregiver support: Access to support services and guidance tailored to caregiver needs.
-
24/7 support access: Connecting participants with a care team member or helpline for assistance.
-
Respite services: Providing temporary relief through eligible in-home, adult day, or facility-based care.
These services are intended to complement the person’s existing medical care and help families navigate the challenges of dementia.
7. How can respite care help family caregivers?
Dementia caregiving often involves supervision, assistance with daily activities, changes in behavior, and increasing care needs over time. Family caregivers may struggle to balance these responsibilities with work, family life, and their own health.
Respite care provides temporary relief while helping a loved one remain in a familiar environment when appropriate.
Depending on the care plan, in-home support may help with companionship, supervision, personal care, and other non-medical daily activities. Respite can give caregivers time to recharge and help make caregiving more sustainable over the long term.
