Hypertension prevention, diabetes, and blood pressure control are just three heart health related issues that Baltimore City seniors struggle with. Heart disease is the leading cause of death for Americans, and Baltimore City is on the initiative to help prevent. Million Hearts Initiative is one such group for advocacy for heart health. Their program provides education for Baltimore City seniors, free blood pressure monitors, as well as screening information.

Other services include medication management, transitional care, safe discharge and transportation from the hospital or rehab facility, Post Op care both short and long term, Pre Operation and preparation help and assistance, transportation and assistance with outpatient medical procedures and surgeries and client monitoring for out of town family members.
