Navigating a dementia diagnosis can feel overwhelming for both individuals and their families. That’s where the GUIDE Program (Guiding an Improved Dementia Experience) comes in. This Medicare-supported program is designed to provide comprehensive, coordinated care for those living with dementia, while also offering much needed support to caregivers.
The GUIDE Program focuses on improving quality of life through personalized care plans, care coordination, caregiver education, and access to resources. It helps ensure that individuals living with dementia receive consistent, well-managed care while also reducing stress on families trying to manage it alone.
While many individuals living with dementia may be enrolled in outpatient therapies, in-home support plays a critical role in day-to-day success. That’s where BrightStar Care can jump in and support.
Our team works alongside your plan of care to provide reliable in-home support. From assisting with daily routines and personal care to reinforcing care plans and providing companionship, our caregivers help create a safe, structured environment in the home. We also help families stay on track and can assist in communicating updates or concerns with loved ones.

Criteria for GUIDE:
- $2,500 allotted for home care per year
- Must be in a minimum of 4-hour blocks of services
- Must have Traditional Medicare Part A & B
- Must have a dementia diagnosis from a treating physician
- Must NOT be residing in a skilled nursing home or enrolled in Medicare hospice
