Pediatric Nursing and Private Duty Nursing at Home in Plano TX | Joint Commission Accredited

If you have a medically complex child in Plano TX — one who has a tracheostomy, home ventilator, feeding tube, seizure disorder, or multiple specialists — you already know that pediatric nursing at home is not a commodity. The nurse in your home becomes an extension of your family's clinical team. BrightStar Care of Plano provides Joint Commission Accredited, RN-supervised pediatric nursing and private duty nursing at home in Plano TX for children from infancy through age 21.

Private duty nursing is what allows medically complex children to live at home with their families instead of in a long-term care facility. It is also what allows parents of these children to keep jobs, sleep, and stay healthy enough to be parents. Our pediatric nursing team is trained specifically for the clinical demands of home-based pediatric care — which is a different skill than pediatric hospital nursing.

What is pediatric private duty nursing?

Pediatric private duty nursing is one-to-one skilled nursing care delivered in the child's home, typically in shift-based coverage — 8, 12, or 16 hours per day, including overnight coverage. At BrightStar Care of Plano, every pediatric nursing case is Joint Commission Accredited and supervised by a Registered Nurse. Our RNs and LVNs are trained in the specific clinical skills that medically complex children require.

Who qualifies for pediatric private duty nursing?

Children with complex medical needs typically qualify — those with tracheostomy, home ventilator, BiPAP dependence, G-tube or J-tube with continuous feeds, central line or TPN, uncontrolled seizure disorder, or multiple co-occurring conditions. Texas Medicaid programs — Medically Dependent Children Program (MDCP), Community First Choice (CFC), STAR Kids, STAR Health — authorize specific nursing hours per week for qualifying children. Private insurance occasionally covers pediatric private duty nursing, usually for short-term post-acute needs.

Clinical Skills Our Pediatric Nurses Provide

  • Tracheostomy care — routine suctioning (oral, nasal, tracheal), site care, tie changes, routine trach tube changes
  • Home ventilator management — LTV, Trilogy, Astral vents; settings verification; alarm response
  • BiPAP and CPAP management — nocturnal and continuous use
  • Feeding tube management — G-tube, J-tube, GJ, NG; bolus feeds, continuous pump feeds, medication administration via tube
  • Seizure monitoring and rescue medication — Diastat, intranasal Versed (midazolam), seizure action plan execution
  • Medication administration — oral, IV, SQ, IM, rectal, nebulized, inhaled, per feeding tube
  • Central line care — PICC, port, Broviac, Hickman — dressing changes, flushes, access
  • Oxygen therapy — concentrator, portable, high-flow
  • Respiratory treatments — nebulizer, CPT, vest (airway clearance)
  • Catheter and ostomy care
  • Post-surgical recovery — incision monitoring, drain care, pain management

Shift-Based Scheduling Options

Most pediatric nursing families run some combination of:

  • Day shift — typically during school hours or when parents work
  • Overnight shift — allowing parents to sleep, which is the single most common request we get
  • 16-hour coverage — day and evening combined, leaving parents with overnight
  • 24-hour coverage — typically with multiple nurses rotating through shifts
  • Weekend and respite shifts — allowing families flexibility

Coordination with Plano Pediatric Specialists

Plano-area pediatric patients are typically treated by Children's Health Plano (formerly Children's Medical Center Plano), Cook Children's, UT Southwestern pediatric specialists, or Texas Health Pediatric Plano. Our RN communicates directly with pulmonology, neurology, GI, cardiology, complex care clinics, and the child's pediatrician. When the child also has a school nurse, we coordinate with the school too.

Paying for Pediatric Private Duty Nursing

Texas Medicaid programs — MDCP, CFC, STAR Kids, STAR Health — authorize specific nursing hours per week for eligible children. We bill Medicaid directly. Private insurance occasionally covers pediatric private duty nursing for specific situations. Our team helps families navigate both Medicaid enrollment and private insurance authorization.
