Skilled Nursing Services We Provide at Home

Wound care and wound VAC management. Our nurses assess and treat all types of wounds — surgical incisions, diabetic ulcers, pressure injuries, venous and arterial ulcers, traumatic wounds. We provide evidence-based dressing changes, debridement, and manage wound VAC (negative pressure wound therapy) devices at home, eliminating multiple weekly trips to a wound care clinic.

IV therapy and specialty infusions. Our nurses administer IV antibiotics, IV hydration, IV diuretics, and other specialty infusions at home. We manage PICC lines, midlines, peripheral IVs, and implanted ports — including dressing changes and routine flushing.

Medication management and administration. We provide comprehensive medication management — verifying the complete medication list, ensuring correct administration on physician-prescribed schedules, documenting every dose, and communicating directly with physicians when concerns arise. Medication management includes oral, injectable, inhaled, and enteral medications.

In-home lab draws and blood work. Our nurses perform venipuncture and phlebotomy in the home — drawing CBC, CMP, INR, BNP, HbA1c, and virtually any ordered lab panel. Specimens are couriered to the processing lab the same day. No clinic travel required.

Ostomy care. Our nurses provide colostomy, ileostomy, and urostomy care at home — pouch changes, skin barrier management, stoma assessment, and patient and family education for new ostomates.

Feeding tube management. We manage G-tube, NG-tube, and J-tube feedings and site care at home — including tube feeding administration, residual checks, medication administration through the tube, and site assessment for infection or skin breakdown.

Post-surgical nursing care. Our nurses provide comprehensive post-surgical monitoring, wound care, drain management, anticoagulation oversight, and coordination with the surgical team for patients recovering from joint replacement, cardiac surgery, cancer surgery, spine surgery, and other major procedures. Chronic disease management. Skilled nursing visits for congestive heart failure, COPD, diabetes, and other complex chronic conditions — monitoring symptoms, managing medications, drawing labs, and intervening early before deterioration leads to emergency department visits or hospitalization.

Pediatric skilled nursing. Our pediatric nursing team provides skilled nursing for medically complex children — ventilator management, tracheostomy care, tube feeding, IV therapy, and private duty nursing shifts — coordinating with Children's Medical Center Plano and other pediatric specialty teams.

RN Supervision on Every Care Plan
