The people on your loved one’s care team can make all the difference. When that team includes a registered nurse, you’ll receive additional care, support, and resources that go beyond your loved one’s day-to-day medical needs.

Prior to start of services, we will send our RN out to perform a preliminary assessment of your loved one to evaluate their care needs. The assessment involves a review of body systems to get a complete perspective on the client’s physical and mental health, as well as a home safety review. This is done on our clients who require skilled services, right down thru clients who “just need a weekly haul to the grocery store”.

Having an initial assessment provides us a baseline so that we know what to expect in caring for your loved one, as well as cueing us in to changes that may need to be addressed. So, when Mom or Dad begins to decline assistance with personal hygiene, for example, our RN can troubleshoot accordingly. Perhaps it’s resistance due to feeling a lack of control. Or it could be some other factor that’s impacting them – like an infection in the body causing a change in mental status. With our clinical expertise, we know to monitor for these conditions.

In addition to gaining insight on the stuff – the physical regimen, the environment of care, the errands – our RN captures personal touch points in each Plan of Care administered by our Caregivers. Does Dad like to lounge in his favorite recliner and watch the news? We want to know about it! Our Plan of Care is not just a task list to execute.

Though our RN will have our Caregivers encourage and assist Dad to get up from the recliner to prevent skin breakdown and help with his circulation, we realize the importance of engaging our clients holistically on the things that matter most to them. In addition to helping with his morning shower, our Plan of Care will include preparing Dad a healthy snack and getting in some news watching together.
