Yes, BrightStar Care of Flower Mound can help with transitional care after a hospital stay or surgery. We provide both skilled medical care and everyday assistance during recovery. Our team can assist with the transition from the hospital or rehabilitation setting to the home and help coordinate the services needed for a safe recovery. 

Depending on your needs, post-hospital or post-surgical care may include skilled nursing, wound care, medication management, physical or occupational therapy, mobility assistance, bathing and dressing, meal preparation, transportation to follow-up appointments, and other personal care services.
