How Our Care Management Works

We offer interdisciplinary care coordination tailored to the needs of each patient, and we work with case managers, insurance providers, third-party administrators (TPAs) and employers to streamline the referral and care process.
When partnering with us, submitting a referral is completing a simple form about the patient and clicking a submit button. Our Central Intake team will take the referral and find an accepting agency for your patient. Eighty percent of referrals are connected with an accepting agency within one hour. We’ll also provide documentation updates within 48 hours after a shift or provide weekly updates, based on what’s deemed necessary.
