AGENCY SERVICES

Yes this is led by our Director of Nursing who is highly skilled in creating personalized Plans of Care focused on a Person-Centered care approach.

Yes if required and with permission with the POA our Director of Nursing will coordinate with the clients PCP.

Yes, our director of Nursing will create a detailed Plan of Care and by law will conduct assessments every 90 days or in the event of a hospitalization and /or change in condition. See prior response concerning Medicare services.

The amount and the structure of care is determined based on need and the family’s requests. We have no minimum.

If transporting a client we would charge the IRS rate of .70 cents per mile if transportation is provided in the Caregiver vehicle. Holidays are billed at 1 ½ times the normal rate.

Yes, this is done prior to hire and annually. If the employee has lived in PA for less than 2 years, we also conduct a full FBI background check inclusive of fingerprinting.

This differs on a case-by-case basis but in general a Care Team is assigned to a client, always allowing for backup.

Yes, since we use a care team approach, we ensure that additional care staff is trained on the specific Plan of Care should an emergency arise.

Yes, Supervisory Visits are required every 90 days for non-skilled cases and every 60 days for Skilled Clients.

Punctuality is managed daily with a GPS tracked AP for Time & Attendance. Late Arrivals and other infractions are documented and addressed by the Client Care Manager. In the event of a Theft or any other client complaint, these are immediately logged in our system using and incident report. Once the matter is investigated by the Director of Nursing following the Joint Commission standards.

We provide Guaranteed Compatibility for all clients. If the client or client family are dissatisfied the assigned resource is removed and replaced.
