Yes if required and with permission with the POA our Director of Nursing will coordinate with the clients PCP.
Yes, our director of Nursing will create a detailed Plan of Care and by law will conduct assessments every 90 days or in the event of a hospitalization and /or change in condition. See prior response concerning Medicare services.
The amount and the structure of care is determined based on need and the family’s requests. We have no minimum.

If transporting a client we would charge the IRS rate of .70 cents per mile if transportation is provided in the Caregiver vehicle. Holidays are billed at 1 ½ times the normal rate.
Yes, we provide 24 hour on call support.
Dependent on the emergency we have detailed policies for all potential emergencies that are part of our Certiļ¬ed Joint Commission operating procedures. These are available upon request
